ENTIRE AGREEMENT
This Agreement contains the entire agreement and understanding among the Parties hereto with respect to the subject matter hereof, and supersedes all prior agreements, understandings, inducements and conditions, express or implied, oral or written, of any nature whatsoever with respect to the subject matter hereof. The express terms hereof control and supersede any course of performance and/or usage of the trade inconsistent with any of the terms hereof.

PARTIES
This Agreement is entered into by and between: One Determined Vegan, LLC (Coach Kelley—AFPA Certified Holistic Nutritionist/Health Coach) and Client

PURPOSE OF THE AGREEMENT:
The Purpose of this Agreement is to enter a coaching relationship between the Coach and the Client, where the Coach will train the Client and cultivate his/her personal, professional and/or health goals. Coaching is partnership (defined as an alliance, not a legal business partnership) between the Coach and the Client in a thought-provoking and creative process that inspires the client to maximize their health and personal potential. It is designed to facilitate the creation/development of personal, health, professional or business goals and to develop and carry out a strategy/plan for achieving those goals.

TERM
This Agreement shall enter into force on the date of the last signature by the Parties. It shall remain in force for a period of __90 Days___
The Coach will be available to Client by email, text and voicemail in between scheduled meetings as defined by the Coach (scheduled weekly check-ins). This Agreement may be renewed only by the written consent of the both parties.

PAYMENT FOR SERVICES
The Client will pay the Coach an amount of __$$$___ for the performance of the services (hereinafter referred to as “the Fee”)
Whereas the Coach will invoice the Client before any official services are rendered.
All Sales are Final.
